N opened 1C, S bid 2D, What should W bid? How do EW get to 4 H? N will support to 3D.

The auction you give is very strange: a 10-point opening and then a jump response - these bids should be alerted. If 1C is natural, East might choose an aggressive 2C Michaels Cue Bid to show both majors - over which West would surely jump to 4H.
